To find your neck size, measure around the base of the neck. When wearing the shirt use the one finger rule, you should be able to place your forefinger comfortably between the collar and your neck.

To find your sleeve length, bend your elbow like you are looking at your watch. Have someone measure from the middle of the base of your neck to your shoulder point then to the elbow and to the wrist bone.
